- A certificate generally lists one or more lines of insurance, the limits for those coverages, and the insurer.1
- The third parties who request certificates are called certificate requestors or holders.2
- The current ACORD 25 warns that the limits shown may be only the amounts the certificate holder asked for, not the full policy limits.3

Pool operator cards and lifeguard certificates.
Where your trade calls for a credential, this is what one looks like.
Certifying bodies each design their own. This mock shows the details a community looks for: who, what, when issued and when it ends.

If you are changing your business type.
What each business type is, with the official page behind it.
The short version
This page describes each type and leaves the choice to you and your accountant.
Sole proprietorship
Partnership
- A partnership files an information return but pays no income tax itself; profits and losses pass through to the partners.1
- A partnership files Form 1065, U.S. Return of Partnership Income.2
- Partners are not employees and should not be issued a Form W-2; the partnership furnishes each partner a Schedule K-1.3
LLC
- LLC owners are called members, and most states allow single-member LLCs.1
- A domestic LLC with two or more members is taxed as a partnership by default unless it elects corporate treatment on Form 8832.2
- A single-member LLC is disregarded as separate from its owner for income tax unless it elects corporate treatment on Form 8832.3
C corporation
- C corporation profit is taxed to the corporation when earned and again to shareholders when paid out as dividends.1

Nonprofit
- A nonprofit must apply to the IRS for tax exemption; that is separate from registering with the state.1

When your EIN changes.
A change of ownership or structure generally calls for a new EIN. These are the IRS’s own pages.
- An EIN is a federal tax ID number the IRS issues to businesses, tax-exempt organizations and other entities.1
- The IRS says a business needs an EIN if it has employees, or will pay employment or excise taxes (or alcohol, tobacco and firearms taxes).2
- Partnerships, LLCs and corporations all need an EIN to operate.3
- You can apply for an EIN online, directly with the IRS, for free.1
- The online tool requires the responsible party's SSN or ITIN, and a principal place of business in the U.S. or U.S. territories.2
- When the online application is approved, the IRS issues the EIN immediately online.3
- You can use a new EIN immediately for most business needs, such as opening a bank account or applying for business licenses.1
- For some uses (e-filing a return, EFTPS payments, TIN Matching) the IRS says to wait up to 2 weeks after getting the EIN.2
- The responsible party is the person in charge of the entity and its assets and must be named on the EIN application.3
VendoRFP never asks you to type your EIN. It lives only on the W-9 you upload, so you can see exactly where it goes.
